Motor Carrier Identification Report as well as an Application for U.S. DOT Number. This collection of information is mandatory and is required by 49 CFR Part 385 and authorized by 49 U.S.C. 504 (1982 & Supp. III 1985). The Form MCS-150, Motor Carrier Identification Report, must be filed by all motor carriers operating in interstate or foreign commerce. A new motor carrier must file Form MCS-150 before beginning operations, Exception: A motor carrier that has received notification of a safety rating from the Federal Motor Carrier Safety Administration (FMCSA) need not file the report.
